Liability Waiver and Release Agreement
By signing this waiver, I acknowledge and agree to the following terms and conditions regarding my participation in escape room activities at this venue.
Assumption of Risk: I understand that participation in escape room activities involves certain risks, including but not limited to physical injury, and I voluntarily assume all such risks.
Release of Liability: I hereby release and hold harmless the venue, its owners, employees, and agents from any and all claims, damages, losses, and expenses arising from my participation.
Medical Acknowledgment: I confirm that I am in good health and have no medical conditions that would prevent me from participating safely.
Photo/Video Consent: I consent to being photographed or recorded during my visit for promotional and operational purposes.
